Ontario concrete company fined for on-site forklift collision

TORONTO ­– Ontario’s Ministry of Labour, Immigration, Training and Skills Development has fined Ilderton, Ont.’s Coldstream Concrete Ltd. $75,000 following a 2022 collision between a forklift and front-end loader that injured a worker.

In a news release from Feb. 27, the Ministry stated that Coldstream failed to take all reasonable precautions in their workplace to protect the injured worker.

The incident occurred after a worker had been operating several lifting devices, including a forklift and front-end loader, to transport concrete products to a casting area. The worker returned to an outdoor yard via forklift to pick up another piece of product and collided with the front-end loader on a ramp leading to the main building, significantly injuring the operator.

A provincial investigation determined that the collision could have been avoided by using a proper traffic management plan. In addition to the fine, Coldstream Concrete was order to pay a victim surcharge of 25 per cent that will be credited to a special provincial government fund to assist victims of crime.
